Transit Bipartition process

In German, transit bipartition means “split bowel” and is a procedure in which the bowel is effectively diverted. First, sleeve gastrectomy surgery is performed. In the next step, a connection is established between the lower small intestine and the lower part of the stomach. The small intestine close to the connection to the large intestine (approximately 80-200 cm proximal) is cut and divided into two parts. It then connects to the upper end—the stomach—the large intestine. This distance may vary from person to person.

Due to intestinal bifurcation of the intestines, food can now pass through the normal (~1/3) pylorus, duodenum, and small intestine, or directly (~2/3) into its lower part. Here, hormonal mechanisms are activated, which both lead to the feeling of early satiety and accelerate the recovery of the metabolic state in the body. This new process does not cause malabsorption, as food is also digested normally!

What is Transit Bipartition?

In transit bipartition (intestinal bifurcation), a gastric sleeve operation is performed in which most of the stomach is removed. A connection is then established between the lower small intestine and the lower part of the stomach without cutting any part of the intestine. Thus, food can go to the stomach in two different ways with this method and generally without any digestive disorders.

How is the procedure done?

First, sleeve gastrectomy surgery is performed. This requires extensive preliminary checking to rule out the presence of inflammation or tumors. The liver, pancreas and gall bladder are also examined. The procedure is performed under general anesthesia and takes about an hour. It is performed as a laparoscopy procedure, which means that several skin incisions of about two centimeters are made where the camera and instruments are inserted. With the incision device, most of the stomach is cut and the formed suture is closed immediately. Only then does true Transit Bipartition begin by connecting the lower part of the stomach to the lower part of the small intestine.

Are there risks and side effects?

Since the intestines can no longer absorb enough vitamin B12 after gastrointestinal surgery, it may be necessary to take this vitamin artificially. The risks of adverse events during and after the procedure largely depend on the patient’s state of health and therefore vary from person to person.

What measures should I take?

You need to change your diet permanently. For this purpose, a regulated nutrition program is recommended together with a nutritional counseling. For the first two weeks after the surgery, you can only eat liquid or light foods. After this process, you can get basic foods. However, you should avoid too much oil. You can divide your meals into 5 to 7 small meals a day.
